This group is a collaborative forum focused on the implementation and operational understanding of hospital at home models within Canada’s healthcare system. It does this by enabling coordination and knowledge sharing on the delivery of acute, hospital-level care in home and residential settings, including clinical, operational, and technology considerations. This matters because hospital at home models offer an alternative to inpatient care that can support system capacity, patient experience, and appropriate use of resources when applied to suitable populations.

Ultimately, this work helps participants align on practical approaches and shared learnings to support consistent, informed adoption of hospital at home services.
