Hello everybody -

I'm a physician who has been practicing in Manitoba for ~ 10 years both in hospital and community settings. I have been pretty frustrated with the state of the available healthcare information systems in my province and have recently moved past the expectation that somebody will eventually be on their way to fix it. I have seen the development of effective clinician resources move at a snails pace (that's being generous) and I feel like I don't see anyone really taking the initiative to move on it. It seems evident to me that very few of the vendors have clinicians directing their development and it feels like it's harming the quality of patient care as well as physician wellness. Records are incomplete, difficult to keep up to date, use esoteric and/or outdated standards/language, none of the systems speak to each other, and most of the patient data ends up getting sent as a pdf which defeats the purpose and power these systems are supposed to bring us. I have to use clunky annotation tools on facsimiles of paper prescriptions sent by our pharmacies, clicking the page sometimes dozens of times to refill a single prescription. Lab results come to me again as faxed pdfs with data that is variably available to the EMR (some results get populated into the lab module, some don't) and the EMR does a piss poor job of trending or showing reliable results in a usable way. Bugs in the infant weight / length module list an infant who is actually FTT (<10% centile) as an error and plots them as >99% centile. These things (among others) are making me tear my hair out. I am actively learning computer programming with the intent to build my own EMR because I can't actually believe how incredibly poor these systems are. Am I alone here? Why is the system so broken? Other clinicians I talk to seem just resigned to working within a broken system and finding their own workarounds, sometimes at the cost of their own sanity or work/life balance. Is anyone on here having a similar experience? I'm looking for some partners to help figure out what solutions there may be. Is this even the right place to have these discussions? I'd appreciate some insight if anyone has any. Hope everyone is staying well!
